Question

I have type 1 diabetes and I take 25 units of Humulin L in morning and 10 units at night along with 5 units of lispro before meals. I seem to be holding my own, but the manufacturer stopped making Humulin L here in Canada so the doctor gave me a prescription for Humulin NPA. I have been reading that the two should not be mixed. Any comment?

Humulin NPH can be mixed with lispro. NPH will have an action similar to that of Lente, but its duration of action is typically shorter so your dose may need to be adjusted.

As I understand it, NPA is only used in pre-mixed insulin.This means that the insulin is already mixed so that for example 20 units of 75/25 would contain 15 units of intermediate-acting insulin and 5 units of rapid acting insulin. You are correct that the manufacturer says not to mix this with other insulins.

You need to clarify which type of insulin has been prescribed, I think you probably also need the assistance of a diabetes team at this point since there are many treatment options available. The Canadian Health System has a multitude of excellent diabetes programs. To find one in your area, contact the Canadian Diabetes Association.
